Windows Phone Thoughts: Last Chance for Thoughts Media Reader Survey 2006

Later today I'll be closing off the 2006 Thoughts Media Reader Survey, so if you haven't taken it yet, now's your chance! If you happened to have difficulty getting the first one to display (some readers saw only a blank page), please give it another go - a helpful community member cleaned up the code and it seems to have solved the problem.
